VITALITY is a private-pay/cash-based clinic, and is out-of-network and non-participating with all insurances.
Payment is due in full on the date of service provided. Please contact Lisa for specific pricing of services.
Cash, checks, and credit cards are accepted, including Health Savings Accounts (HSAs) and Flexible Spending Accounts (FSAs).
If you have out-of-network benefits under your insurance plan, a Superbill or Summary Statement of Services can be provided for you to submit to insurance for possible reimbursement. Self-claim submission is the patient's responsibility.
Cancellation Policy:
VITALITY requires a minimum of 48 hours notice for all cancellations to avoid being charged in full for the missed appointment. This is to allow ample time to schedule someone else who is in need. More advanced notice is greatly appreciated!
